Aging Solutions supplies this vital component of care. Our experience with both the elderly and dependent adults gives us a unique skill set: overseeing and ensuring that the personal and health care needs of clients are fulfilled while also meeting rigorous standards for accountability to financial institutions and the courts.
Aging Solutions understands your responsibility for the continuing financial vitality of the person under your care, and for ensuring a high quality of life as well. We act as your eyes and ears, supervising, overseeing, and providing the checks and balances needed for a continuous high quality of lifefor your clients.
We don’t act as conservators, fiduciaries or trustees. We also don’t take kickbacks or referral fees from nursing homes or other providers, so that our recommendations are independent, objective, and in the best interests of your clients.
Aging Solutions provides a team approach to client care that includes a nurse-care manager to provide onsite medical assessments and to connect to a client’s health team, and a non-clinical care coordinator to oversee all aspects of a client’s needs, including:
- Ensuring that medical and other appointments are kept, and that health care is appropriate, thorough, and cost-effective;
- Identifying financial exploitation or its potential for your diminished-capacity clients, helping to make them whole, and eliminating future abuse;
- Helping identify and provide access to all physical, mental, educational, or recreational services available;
- Assisting with inventory of current assets with a dual inventory system, to build a baseline for use in any future administrative or civil proceedings;
- Identifying administrative cost savings and streamlining procedures;
- Ensuring that all benefits to which a client is entitled are being accessed;
- Providing independent, objective oversight of personal care services and household maintenance needs.
