Initial Consultation
Before an initial consultation, Aging Solutions collects a brief summary about a new client's situation, derived through the original phone call or email. We make it a point to prepare both informational and educational materials that may pertain to your situation before the consultation begins. We use standardized interview tools which we have modified to ensure a comprehensive evaluation by the end of the consultation, as well as appropriate education for you, and tangible solutions.
During the initial interview, we identify current problems, possible solutions to those problems, and potential roadblocks. At all times, our approach is holistic: we look at the entire situation. A seemingly simple question about a new living situation necessarily requires answering about financial and medical questions.
Once we have identified the critical elements of your situation, we educate you about your various options, and help devise both short-term and long-range plans that will accomplish objectives in the best interests of the family and the older person.
This process generally takes from sixty to ninety mintues. Clients who are directly served in a consultation include the older person on his own, spouses, adult children, or some combination of all of these. Often, when multiple family members are involved, we coordinate a conference calls to save time and to provide everyone the same information at the same time. These appointments generally last longer because more participants usually means more questions.
Ongoing Support
After the initial consultation, Aging Solutions sets up an account for each client in our database. Once the client record is established, we can provide ongoing assistance.
If an action plan has been established at the initial consultation, the client family can carry out the plan, or request further services from us to perform the necessary tasks. These may range from making a simple phone call or writing a letter, to making contact with an appropriate professional for specialized services, to visiting a nursing home or other facility. We are always there to troubleshoot whenever the initial situation changes or when a crisis occurs.
